  Transmittal Letter - currently dated and signed by an authorized officer of the FTZ grantee corporation. 

  Executive Summary

  Exhibit 1 - Legal  Authority to Apply 
	a.	Reference to original subzone approval order and any expansion orders
	b	Indication that all relevant legal authority remains in effect	

  Exhibit 2 - Site Description, either: 
	a.	Brief description of existing site where new activity will take place and 
		how the site will be modified, or 
	b.	Information on any new site, including legal description, ownership/right 
		to use, maps, address, acreage, type of activity currently taking place 
		on the site...
	
  Exhibit 3
	a.	Statement on Customs automated system and which Port of Entry is 
		monitoring subzone
	b.	Time schedule for project completion activation
	c.	Operational plan

  Exhibit 4
	a. 	Brief description of company, existing facilities and activity   
	b.	Project Information:
		-	Description of expansion or new facility construction and type of units 
			being added, reason for investment, any new product lines
		-	Current and proposed crude distillation capacity
		-	Current and proposed feedstocks and finished products  
		-	Indication of where feedstocks are sourced and if this will change
		-	Export information, direct and indirect
		-	Current and proposed employment (direct and indirect) 
		-	Contract employment/investment in construction of new facilities
		-	Estimated U.S. value added at facility
		-	Types of customers
		-	Estimated increase in savings 
		-	Company's share of domestic market
		-	Impact of increased zone savings on domestic industry
		-	Current industry trends
		-	Why is an expanded scope of authority in the public interest
	c.	Environmental Impact
		- 	Indicate local zoning and status of any applications for construction 
			approvals. 
		- 	Are there any land-use issues? 
		- 	Will the expanded use of FTZ procedures change the physical 
			aspects of the plant or its operation? 
		- 	Is FTZ status or the use of FTZ procedures a requirement or a 
			precondition for the proposed activity or construction at the plant?

	-	Include maps if requesting additional sites
